Please provide ESTIMATES to the percentage of services provided within the surveyed facility. The following three scenarios are examples of ways to describe a facility: 1) a facility that is comprised of a hospital only would be entered as: Hospital 100%; 2) a facility that is comprised of both long term care (nursing home) and hospital services equally would be entered as: Hospital 50%, Long Term Care (Nursing) Home 50%; 3) a facility that is comprised of hospital, long term care, and clinic services could be entered as: Hospital 40%, Long Term Care (Nursing) Home 40%, Clinic 20%. Again, the information is the facility's ESTIMATE of the offered services. This information will be used to develop an estimated picture of the service scope offered in hospital facilities. |

The survey fields required for this area of the survey are: "Number
of Positions (Start of Semester)," "Number of Positions (End of Semester)," and "Number
of Positions that left during the semester." Employee figures are not
asked on FTE level, rather the number of positions employed be it full
or part
time. The survey requests position numbers instead of employee numbers
for instances in which organization employs many people who hold more than
one position. If your organization does not employ a great number of people
with more than one position you may choose to enter employee numbers instead.
For purposes of this report, the start of the semester can mean the first
day of the semester or the closest pay period date in the surveyed facility.
The end of the semester can mean the last day of the semester or the pay
period date closest to six months from the starting date. All other fields
are optional, including the entire column of Turnover Cost, provided to
capture a more comprehensive perspective of turnover within hospital facilities.
"Leaving" an organization can occur from several reasons including termination
(voluntary or nonvoluntary), retirement, or death. Also, an employee who switches
facilities for a system should be counted as a termination for the left facility
and a new hire for the new facility.
The formula used to automatically calculate turnover is the average between the "Number
of Positions" at the end and beginning of the measuring period divided into the "Number
of Positions Vacated." For example, if the surveyed facility had 100 employees
at the beginning of the semester and 98 employees at the end with 4 employees
that left (hypothetically meaning two of the 4 vacancies had been filled), the
formula would appear as (100+98/2) = 99; 4/99 = 4%.
Note: It may be a practice in some organizations to allow employees to transfer to an employment status that is a "very limited working status." This may be called "on call," "casual," etc... Due to the myriad of definitions of such employees we ask that these numbers be excluded from this survey.
Additionally, for purposes of this survey if an employee chooses to become "casual" he/she should be considered terminated. Likewise is a casual employee that becomes full or part-time, that employee should be counted a new hire. |

For example if your organization employs 50 CNAs, this area of the survey will calculate your total CNA turnover rate. If your organization does not employ a position listed simply enter zero.
