*Reimbursement will be based on up to half of the intern's hourly wage. Reimbursement totals are calculated in two stages: (1) upon the April 18 deadline, all organization applications (this form) are reviewed for that year's reimbursement allowances (traditionally this allowance was a maximum two students per organization for reimbursement up to half of $9/hour) - these are determined via application numbers, student numbers and student hours requested, if possible all applicants will be granted at least one student; (2) upon completion of the program (Labor Day) all reimbursement requests are submitted and reviewed to ensure compliance with the grant agreement. Because some students and organizations cancel SHCIP during the program's timeframe there are usually funds left over at the end of the program. After an unused fund distribution formula has been agreed upon by the SHCIP Advisory Group, the Minnesota Department of Health (MDH) is invoiced. Once MDH completes their review process, funds are distributed to the SHCIP financial coordinator, subsequently SHCIP employers are reimbursed. Because of this two-phase process, oftentimes the reimbursement amount first granted under the SHCIP contract is not what is actually reimbursed. If employers fulfill their original allowance of both student numbers and hours this amount will likely be more than the original reimbursement amount. If employers do not fulfill the original allowance of both student numbers and hours this amount will likely be less than the original reimbursement amount.

**Students must work/observe a minimum of 100 hours to a maximum of 480 hours.
